cottongin f967575ebc feat: add native macOS overlay window with pulsing border 
Uses NSWindow + PulseBorderView with Core Animation-style pulse.
Fixed NSRect handling from plan: uses NSInsetRect instead of
non-existent .insetBy method on pyobjc tuples.
